This week’s assignment is a PPT about Evidence-Based Practice Model and Change Theory. Create a PowerPoint presentation of 8-14 slides that addresses the following points/questions. PPTs should include a title slide, objective slide, content slides, and a reference slide. The sources should be clearly cited on the slides where used in APA style. Use clear headings in the PPT that allow your professor to know which bullet you are addressing on the slides in your presentation. Support your content with at least three (3) sources.
